In 1997, the Institute of Medicine defined computer-based patient records (CPR) as 'essential technology' to practice medicine [R]. The evidence indicates that electronic patient records has the potential to improve quality of care. The CPR has been known in different countries in different times and circumstances as the electronic medical record (EMR), electronic patient record (EPR), electronic health record (EHR), personal health record (PHR) and integrated care record (ICR), etc.
